Many “talk the talk” of moving care out of the acute environment and into the home and the community. Many useful eHealth solutions have emerged, and some implemented, that enable new models of care outside acute institutions. These have included systems that are affordable by Long Term Care facilities and yet support more efficient clinical and operational processes, mobile systems that can accompany care providers during home visits and be interoperable with central systems, and systems that can be used directly by patients at home. This session addresses this challenging area by looking at what we have that we can build more effective practices on, and what we still need in order to achieve real impacts. Unless we adopt enabling technologies in this sector of care we will never “walk the walk”!
